On this page you will find links to useful documents for current and incoming patients. For example, the authorization to release protected health information is necessary for me to communicate with your other health care providers, family members, or other individuals whose input would be valuable to your care. When you make your first appointment, you should receive a link and a text message to our secure electronic health records service where you can enter all of your relevant new patient information. We will also be updating this site periodically with helpful information about things like co-pay cards to help with higher co-pays on certain medicines.
